This is a short statement confirming a student has achieved the required elements to register with the HCPC.

• Digital or Hard Copy Placement Transcript.
Listing location of placement and total hours. Please note for older records this may not be available.

• Completion of Forms.
Purchase this product for completion of Health Sciences related forms such as CGFNS, NNAS, PBHK, CAPR.
Please email copies of relevant documents to student.records@uea.ac.uk once purchase is complete.

Please note that this process will take 8 weeks to be completed due to the complex nature of the form, manual processes and the different departments involved. The 8 weeks starts from the date we receive all information required to complete the process.
